Is the medical intake form required?
Yes! All new medical patients are required to complete a patient intake form before their first medical purchase at High Haven or Aces Dispensary. To save time, we recommend completing the form online before your visit.
Can I complete the form at the dispensary?
Absolutely! You can complete your intake paperwork in person when you arrive. However, filling it out online ahead of time helps streamline the check-in process so you can spend less time on paperwork.
Can I complete the form on my phone?
Yes! Our online medical intake form is mobile-friendly and can be completed using your smartphone, tablet, or computer. Simply click the “Complete Medical Intake Form” button to get started.
Do I need to complete the form every visit?
No! The intake form is generally only required for new medical patients. Once completed, you won’t need to fill it out for every visit unless your information needs to be updated or additional documentation is required.
Do I still need to bring my medical card?
Yes! Even if you’ve completed your intake form online, you’ll still need to bring your valid medical cannabis card or other accepted medical registration documentation, along with a government-issued photo ID, for verification at check-in.
What happens after I submit my intake form?
Once you’ve completed and submitted your form, your paperwork will be available for our team to review, subject to successful submission and receipt. When you arrive, we’ll verify your medical cannabis credentials and identification to complete the check-in process.
